This is the form that Student Telecommunications would use when an employee does not have two forms of picture identification.

The department head must accompany the employee to StuTel and complete the bottom half of this form.

    I played around with the formatting a little and changed "additional" to "addition" in the very first sentence. I uploaded my revised version.
